When you call to schedule your initial visit, please be prepared to provide us with the following information: full legal name, phone number, problem area to be treated, insurance provider, and referring physician, if applicable. Please remember that you do not need a physician’s referral to be evaluated. For more information, click here.
Because we see our patients one-on-one and try to keep you with the same therapist each visit, we suggest scheduling ahead to maximize your ability to get appointment times that work best with your schedule. We will do our best to accommodate your scheduling needs. Please let us know if you are a post-surgical patient.
Your First Visit
We ask that you come 15 minutes early to your first visit to make sure you have all of the forms filled out and that you are ready at your scheduled time. Because of the nature of our one-on-one appointments, we run on time throughout the day. We don’t want precious moments of your appointment time taken up with paperwork.
We recommend wearing/bringing loose clothing that can expose the body part that we are evaluating. We do have gowns, extra shorts, and draping sheets to keep you modest. Please bring supportive shoes for exercise.
We schedule 45 minutes to 1 hour for evaluations. Plan to be here up to 1½ hours in case the therapist recommends a modality for afterwards.
Bring your driver’s license, insurance card, and a list of current medications (including over-the-counter, supplements, and vitamins). Please provide your dosage, how frequently you take each medicine, and how each medicine is administered.
We have a patient portal for which we highly encourage all of our patients to sign up. This portal, called Keet, allows you to receive exercises for home performance in both video and written form, communicate with your therapist through a chat function, and complete important paperwork. When you schedule your first appointment, we will ask for your email address, then you will receive an email invitation from Birmingham Physical Therapy to sign up for the portal. Please sign up prior to your first visit and complete the paperwork that has been assigned to you. If you have difficulty, we will help you register when you come for your initial appointment.
Please fill out the following forms and bring them to your first visit. If you do not have access to a printer, please arrive early to complete them in the clinic. If you are insured by a company not listed above, please give us a call to verify if we are providers with your carrier. We may be able to accept plans not listed above.
At your first visit, we will gladly call your insurance carrier to verify your benefits. While we do our best to insure that this information is correct, it is only an estimate of your responsibility. We encourage you to also check with your insurance company to verify the information they provide to us.
We also offer private pay rates for physical therapy treatment. Please call or stop by the clinic to discuss these options.
Dry needling is covered by BlueCross BlueShield of Alabama in most cases. Ask your PT for more information about this treatment option.
